What receipts are acceptable for payment?

We accept only original receipts (except in a coordination of benefits situation with another carrier) that includes the provider’s name, the amount paid, the balance owing, and the date. We do not accept cash register receipts, canceled cheques, credit card receipts or statements, or balance due statements.
