How do I submit a claim?

Choosing a In-Network or Participating Provider from our provider networks eliminates the need for you to submit a claim. However, if you choose to receive care from a “Non-Participating Provider”, you will have to pay the provider in full for services and subsequently submit a claim to us for reimbursement at a reduced percentage/amount. Individual Medical policyholders and Group Plan Members are required to register for My Colina portal at www.colina.com and submit claims using the “Submit a Claim” feature.
