Please have our sympathy on the passing of your loved one. In order to process the death claim, we will need the below forms to be completed and required documents submitted. Copies can be submitted via email to allow us to begin processing the claim. However, it is mandatory that we receive original copies of these forms along with original identifications before the claim is finalized.

General Guidelines for Submitting Death Claims This outlines the forms and documents that must be submitted to process the claims.

Claimant’s Statement A separate form must be completed by each beneficiary.

Physician’s Certificate The must be completed by the deceased client’s physician.

W-9 form This form must be completed if either beneficiary is a resident or citizen of the USA.
